• Must be an active member of VSHP with at least 5 years of sustained commitment or contribution to excellence in pharmacy practice. and at least 10 years of licensure as a pharmacist.

VSHP Collaboration Award

• Presented to a group of healthcare stakeholders who have made a recent and significant contribution (within the last two years) to the improvement in pharmacy services and community health within the Commonwealth of Virginia spanning multiple health care entitites (ie: multiple health systems, educational organizations, public health, schools of pharmacy, Board of Pharmacy)

• The contribution shall involve public visibility in some capacity 
• Contributions can represent the innovative efforts of an individual or a group of individuals (several individuals or a department)

• Collaboration with non-pharmacy clinicians is encouraged
• Pharmacists and technicians must be VSHP member(s) to be eligible for the award

• Non-pharmacy clinicians do not need to be VSHP members to receive the award but must be nominated in conjunction with a VSHP member.
